GRADE SUBSTITUTION FORM

To change your major into Computational Media, you will need to download this form, get a signature of your current advisor, and sign up for a change of major meeting here.
To change your major out of Computational Media, please set up an appointment in the advising scheduler. Click here to start an appointment.
To add a minor you will need to meet with the CM advisor as well as the advisor in the department of your minor to get signatures for the change/declare minor form.
PETITION TO THE FACULTY
A “Petition to the Faculty” form may be completed by a student when asking for an exception to an Institute rule or policy. Some examples of exception requests include:
- To withdraw from school past the deadline
- To be readmitted after being placed on DROP status
- To be granted an individual course substitution
All “Petition to the Faculty” forms must be completed by the student and signed by all appropriate offices.
If you have any questions or concerns regarding completing the form then you can direct those to the Assistant Director of Freshman and Sophomore Initiatives in Undergraduate Advising, Wes Kirkbride. You can email him at email@example.com.
Once the petition is completed, you will need to meet with the Director of Undergraduate Advising in the College of Computing, Katie Raczynski. You can email her at firstname.lastname@example.org to make an appointment.

UNDERGRAD DEGREE PETITION
As a graduating senior, you must complete the Online Application for Graduation (OAG).
The OAG is only open during certain times of the year based off of your graduating term. Spring graduates must complete the OAG in Sept/Oct, Summer graduates must complete the OAG in December/January and Fall graduates must complete the OAG in May/June.
You will find the specific dates for the OAG window in the academic calendar at the registrars website here.
Before your corresponding OAG time window opens, you will receive detailed instructions via email from your academic advisor. This email will have details on how to complete the OAG information, how to make an appointment with your advisor and a link to complete a CM exit survey.
